reception desk between 8.30am and 6.00pm on weekdays only.
Where possible, please try to see the same doctor for
follow-up appointments in any particular episode of illness.
When your usual doctor is not available you may be seen by
one of the other doctors working in the practice or by a
locum.

We ask our patients to come to the surgery
whenever possible where the facilities are better. If you
feel unwell or are bringing a child who is unwell please let
us know and we will arrange to see you with the minimum of
delay once you reach the surgery. If you need the doctor to
visit please telephone between 8.30am and 10.30am giving
simple details and some indication of urgency when you feel
able to do so.

 | The doctors will be pleased to talk to
you on the telephone if you have a problem. The
receptionists have been asked not to interrupt consultations
except in an emergency. Usually the doctors will be
available to take telephone calls between 11.00am and
11.30am and you book a telephone appointment which are
available at the beginning of afternoon surgery. You can
also contact the doctors by email on the addresses located
under their names on the
The Doctors page. They
will respond as soon as it is practicable to do so.

Register At
The Surgery? |
|
|
|

Anyone wishing to register at the surgery
should telephone or present in person at the surgery during
surgery hours and ask to speak to a member of the reception
team. You will be asked to complete a patient questionnaire,
which will also ask if you have a preference of
practitioner. It will help the process if you have the
details of your last GP and your NHS number, but this is not
essential. You will be offered the opportunity to book an
appointment for a new patient health check, either with you
preferred practitioner or one of the GPs if you have no
preference. Reception staff will be happy to assist with any
queries you might have.

 | If you are on regular medication you can
re-order by the following methods:

3. By calling in at the surgery
(Please use the prescription request postbox on the outside
wall next to the front entrance to the surgery.)

Your request will be recorded;
please follow the instructions carefully. Please speak
clearly, leave your name (or the name of the person for whom
the medication is required), date of birth, address and
contact telephone number in case of a query. Give clear
details of the medication you are requesting and spell the
name if necessary. We recommend that you use the list
supplied with your prescriptions as this gives the correct
name for each medication. |
Requests will be regularly
downloaded and you will be contacted if there are any
queries. Please give at least two working days' notice for
your prescription to be prepared. Requests ordered after
1.00pm on Friday may not be ready until the following
Tuesday. Prescriptions may be collected from the surgery
between 8.45am and 6.00pm or from the Post Office in Milton or
the Village Shop in Ascott by previous arrangement.
If you have run out of your medication or need
to discuss your medication urgently please telephone the surgery
and ask to speak to the dispenser and they will try to help you
or discuss with the Duty Doctor if relevant.
Dispensary This is a dispensing practice where the
majority of prescriptions are stocked in our dispensary. The
dispensary is normally open during surgery hours.

Obtain Test
Results? |
|
|
|
 |
The result of the cervical smear test will be
sent to you by post as soon at it becomes available. For the
results of other investigations please ask the doctor or
nurse when
they are likely to be available and contact the surgery at
the appropriate time. Obtaining these results is the
responsibility of the patient. It would be appreciated
if calls for results could be made after morning surgery. |
